Polypharmacy and resulting inappropriate medication use among frail, seniors continues to be a concern in our healthcare system. The issue of unnecessary medication use is even a bigger concern in the post-acute care settings. Studies highlight the value of interprofessional approaches that engage consultant pharmacists and other healthcare team members in polypharmacy review. Unfortunately, results are hard to duplicate in real-life settings. In this session, the speakers will present a pragmatic model of interprofessional collaboration to operationalize meaningful deprescribing in a large skilled nursing chain that resulted in significant decreases in use of unnecessary medications.
