Skilled Nursing Facility (SNF) and Long-Term Care (LTC) providers have a myriad of current options to directly engage in the pursuit of value-based care. This session, moderated by Brian Fuller, will explore the full spectrum of programs such as Accountable Care Organizations (ACOs), health plan structures such as I-SNPs and strategic partnerships available such as provider-owned networks to decipher how provider organizations sift through the options. This session will discuss key questions, decisions and scenarios providers will need to evaluate in order to ultimately decide which path is best for their organization. In addition, panelists will discuss the broad spectrum of partnership structures, ways to engage and the financial risk profiles to consider across the options. The panel will also predict “what’s next” for value-based care to ensure session participants are thinking about not only today’s, but tomorrow’s options as they evaluate their best value-based care course ahead.
