Depending on who is judging, patient outcomes can take on many different forms and meanings. Patient-reported outcomes (CORE-Q/PROMIS), Family reported outcomes (CORE-Q), Clinical Outcomes, 5-star Ratings, and Value-Based purchasing are various lenses through which patient outcomes may be interpreted. As we work towards providing the best outcomes for all parties are you prepared to be judged? Whether it is clinical or patient-reported, how are your facilities ensuring the best quality outcomes are happening at every level? This presentation will review outcomes, explain the impact outcomes have on your patients and organizations and help you implement solutions for what is sure to be an outcomes-based healthcare future.

Learning Objectives:

  • Define Post-Acute outcomes from various perspectives including patient-reported, clinical, and regulatory.
  • Summarize how outcomes impact various healthcare stakeholders and drive the decision-making processes related to clinical pathways, reimbursement, marketing, quality measures/care compare/ nationally reported data, and healthcare consumerism.
  • Integrate processes for improved outcomes in Post-Acute care including improved employee satisfaction, patient satisfaction, accuracy in outcomes scoring, and data collection.
